Requires legislative approval for Medicaid state plan amendments/waivers that expand coverage or increase state costs. Authorizes specific reforms including rural emergency hospital payments, cost-sharing, comprehensive managed care, Medicaid expansion work requirements, practice authority protection, and site-neutral payments. Repeals/voids certain existing rules and programs.
